What Is Health Anxiety?
Health anxiety is an excessive and persistent preoccupation with having or developing a serious illness, despite reassurance and medical evidence to the contrary. Health anxiety — historically called hypochondria or hypochondriasis — is now understood as an anxiety disorder rather than a personality flaw. Health anxiety affects approximately 4-5% of the population and is characterised by misinterpreting normal bodily sensations as signs of serious illness, excessive checking behaviours (self-examination, internet research), reassurance-seeking from doctors and loved ones, and significant distress and functional impairment despite normal medical findings. Health anxiety is a genuine mental health condition that causes significant suffering and warrants specific treatment.
Health anxiety is maintained by a vicious cycle. Anxiety about health leads to hypervigilance to bodily sensations, which increases awareness of normal sensations that would otherwise go unnoticed. These sensations are catastrophically interpreted in health anxiety, increasing anxiety further. Health anxiety reassurance-seeking provides temporary relief but ultimately increases anxiety by reinforcing the belief that there is something to be worried about. Avoidance of medical information and situations in health anxiety similarly provides short-term relief while maintaining long-term health anxiety. CBT for health anxiety directly targets this cycle.
CBT for Health Anxiety
CBT is the most evidence-based health anxiety treatment. CBT for health anxiety includes: cognitive restructuring of the catastrophic interpretations that drive health anxiety, graded reduction of reassurance-seeking behaviours that maintain health anxiety, attention retraining away from symptom monitoring, and exposure to feared health anxiety situations without reassurance. CBT for health anxiety typically requires 12-16 sessions and produces significant health anxiety symptom reduction. NHS talking therapies provide CBT for health anxiety through self-referral — a health anxiety self-referral form typically takes 15 minutes.
